If you've lost your certificate of failure, please request a copy through the 'contact us' form below and we will send it to you.

Certificate of competency

If you've lost your certificate of competency, you can request a duplicate copy by:

1. Downloading and completing a Statement of loss form 
2. Having the completed form certified and stamped by An Garda Síochána.
3. Posting the form to: The Driver Testing Section, RSA, Moy Valley Business Park, Primrose Hill, Ballina, Co. Mayo, F26 V6E4.
