Please contact your IBT Instructor who will tell you the replacement logbook number that will be issued. This number must be included on the Lost IBT Learners Logbook Declaration Form which you need to sign in the presence of a Garda who will then sign and stamp the form. 

Once completed, send the Declaration Form to: Driver Education, Road Safety Authority, Moy Valley Business Park, Dublin Road, Ballina, Co. Mayo, F26 V6E4.

You can continue your training in your replacement logbook. However, please note that your IBT instructor can only stamp and certify the replacement training entries originally made by themselves.
